International Cooperation
DEEP-RAFT’s overall goal is to create an integrated, hands-on education and outreach endeavor that:
a) Educates doctors on contemporary Atrial Fibrillation and Thromboembolism research, diagnosis and treatment;
b) Empowers the patients to maintain healthy lifestyles, and make accurate decisions for doctor visits and lifestyle changes based on contemporary technological means, such as wearables and digital media and their active involvement in the design of care pathways based on the collection in real-time of their care journey experience;
c) Affects systemic change regarding AF/VTE in the healthcare systems of involved countries, both implicitly through education and awareness, and through active participation of stakeholders such as scientific societies, academic institutions and policy makers to key design and dissemination activities of the project. Several Plan-Do-Study-ACT (PDSA) iterations of improvement actions will take place in small scale pilots, based on actionable metrics that will be recorded by shadowing patients and implementing local working groups and consultation workshops involving practitioners and local policy makers.
